Each month, the M1C Care Team will reach out to targeted Members, especially those at high-risk and who have not had an annual physical in the last 12 months, and facilitates deeper engagement in the health care process. You receive scheduled reports showing how PHM is improving your Members’ health and reducing the risk for large medical claims in the future.
